Pike High School Clinic

Pike High School has a clinic with a registered nurse during school hours. If a student becomes ill, they should request to go to the clinic. Parents will be notified by the clinic in case of serious illness or injury, and arrangements will be made for students to be picked up or sent to a hospital of the parent's choice. Only minor first aid services are available in the clinic.

The clinic will monitor medication administration as well as maintain student clinical records. Please direct questions and or concerns to our nurse at (317) 387-2611 (PHS) or (317) 347-8613.


School Immunization Requirements

In accordance with the State of Indiana, all prescription and non-prescription medications must be administered to students in the health center by designated school employees.  In order for medications to be given to your child, your written permission must be given to the school when the medicine is handed to or sent to the school.  We recommend that you send to the school only the amount of medicine to be given during school hours for the total number of days the medicine is to be given.

The following permission slip has been devised as a way to help you and the school comply with this State requirement.  All blanks must be answered and the form must be signed and dated to be valid.  This form must be completed for every medication sent to school with your child.  You may get the forms from your child’s school office.
